What paperwork do new patients fill out at ANEWU?
New patients at ANEWU complete three things: a medical and dental health history with a current medication list, consent for your exam and imaging, and insurance or payment information. All three are filled out at the front desk on the day of your free consultation, with a team member on hand.
None of it is long, and none of it decides anything about your treatment. The health history exists so your surgeon knows what is already going on before a 3D scan is taken, which matters more in oral surgery than it does at a routine checkup. ANEWU does not offer hygiene or root canals, so there is no cleaning schedule or recall form buried in the stack.

What should I bring to my first visit?
Bring a photo ID, your medical or dental insurance card if you have one, and a current list of your medications. If another dentist referred you, bring the referral slip and any recent dental x-rays. Everything else is completed at the ANEWU front desk when you arrive.
The medication list is the one people forget, and it is the one that saves the most time. A photo of the labels on your phone works as well as anything written down. Old x-rays are useful context, though new 3D imaging is taken in office anyway as part of 3D imaging and guided surgery.

If you have dental or medical insurance
Bring the card itself rather than the numbers copied out. The front desk takes the carrier, member ID and group details straight from it, which is faster and avoids a typo on your chart.
If you are paying without insurance
Leave the insurance section blank. Costs and implant financing options such as CareCredit and Proceed Finance are reviewed at the end of your consultation, once you know what your case actually needs.
If another dentist referred you
Bring the referral slip and any x-rays that came with it. Practices can also send a case straight across through the referring doctors page, and no referral is required to book.
